Safety & Quality Information System (SQIS) RL6
Overview
SQIS (Safety & Quality Information System) RL6 is the Incident Reporting (IR) system for UCI Health. The purpose of IR is to collect patient safety event data to help identify areas of improvement with patient care (such as avoiding medical errors and anticipating potentials problems before they occur, etc.).
Accessing the Application
Step 1:
Go to the UCI Health Intranet site: https://intranet.health.uci.edu/
Step 2:
Navigate to the following on from the menu bar at the top: Administration Tools > Administrative > Safety & Quality Information System (SQIS)
Step 3:
Click on “Log into SQIS”.
Step 4:
Login with your UCInetID and password.
Step 5:
You will be taken to the home page where you need to select the type of form (Incident) you want to document/report. The home page will look like this:
